Full job description
CenterWell Home Health seeks a compassionate Home Health Physical Therapist to deliver one-on-one, in-home rehabilitation services. In this role, you will perform comprehensive assessments, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, function, and safety. You will educate patients and caregivers, complete accurate EMR documentation, and collaborate closely with an interdisciplinary team.
