Speech Therapist

OneHome is seeking a compassionate Speech Therapist to provide high-quality home health services. In this role, you will perform comprehensive speech, language, cognitive, and swallowing evaluations for patients in their homes, creating individualized treatment plans that support meaningful functional goals. You will deliver evidence-based interventions, educate patients and caregivers on home programs and safety, and maintain accurate, timely documentation. Working closely with an interdisciplinary team, you’ll coordinate care, communicate progress, and adjust plans as needed to optimize outcomes, independence, and quality of life in the home setting.
