Full job description

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist role in Fort Smith, AR where you will help patients recover from injuries and manage chronic conditions in a home health setting. This position requires home health and OASIS experience and involves providing care across the agency‘s service area. A recruiter will contact qualified candidates with more details.

Minimum Qualifications

Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program. Active Arkansas physical therapy license or ability to obtain state licensure prior to start. Home Health experience required. OASIS experience required. BLS/CPR certification required. Ability to complete patient evaluations, develop plans of care, and document per state and CMS requirements. Experience coordinating care with physicians and interdisciplinary teams. Ability to travel within the Fort Smith service area and to nearby agency locations as needed.

Preferred Qualifications

Experience with HCHB (Homecare Homebase) electronic documentation system preferred. Previous supervision of PTAs or home health aides preferred.

Assignment Details

Shift hours: 20:00–04:00. Weekly hour guarantee: 32 hours per week. On call/weekend requirement: no. Schedule flexibility and participation in occasional weekend or holiday rotations may be requested to meet patient needs. Mileage reimbursed between visits from first patient to last patient. Coverage area: Fort Smith and surrounding areas. Candidate must be able to travel within the service area and to nearby agency locations as needed.

Impacting Patient Care

Provide home-based physical therapy services following state law and CMS documentation standards. Evaluate functional status, develop and implement individualized plans of care, monitor patient response, and communicate changes to nursing and physicians. Complete progress notes and discharge summaries per agency policy. Participate in staff meetings, in-service education, and maintain current professional licensure and clinical competencies.
