Job Description:

General Summary:

The Advanced Practice Provider – Outpatient  is responsible for the comprehensive medical management of assigned patients. The APP is expected to exercise prudent judgment in the assessment, diagnosis, and implementation of care and seek appropriate consultation with physician as necessary.

*This full-time Advanced Practice Provider - Outpatient role will support Malignant hematology (area of specialty). This will be working days from 8:00am to 6:00pm. This is a 40 hour/wk position working 4 (10 hour shifts/week) working at our LHCP site. Schedule will include some weekend and holiday coverage needed. 1 weekend every 12 weeks and 1 holiday every other year.*

Desired/Required Qualifications:

Education:  

  • Certified and licensed as a Nurse Practitioner or Physician Assistant

  • Holds current BLS card for Health Professionals

  • Hematology/Oncology experience preferred

Primary Duties and Responsibilities:

1. Obtains comprehensive histories and physical examinations that reflect the acuity and presenting complaint of the patient.  Document/dictate progress notes that appropriately capture the content of the obtained history and physical.

2. Functions in the role of an APP to coordinate and deliver comprehensive medical care including following treatment protocols and pathways, writing orders/medications to support patient’s plan of care, ordering and interpreting laboratory and radiographic studies, and seeking consultation from attending physicians as needed to achieve optimal patient outcomes.

3. Collaborates with support staff and nursing teams to optimize patient care outcomes effectively.  Acts as a resource to nursing, support staff and students appropriately.  

4. Communicates in a timely and effective manner with supervising physician(s) as needed for continuity of care and optimal patient outcomes.

5. Serves as a patient advocate and provides patient education and/or referrals for optimal outcome and continuity of care.  Provides effective communication patient/family members that demonstrate sensitivity to their individual needs and concerns.

6. Performs other tasks, procedures, or duties within his/her scope of practice as assigned.
