Nurse Liaison
We are looking for a special leader with Referral management and nursing experience for a Nurse Liaison position. This position requires a minimum of two years of nurse liaison (LTAC, SNF, HHC, Hospice) or skilled nursing referral management experience and a current Michigan RN or LPN license.

Nurse Liaison will be responsible for:
* Identifying admissions appropriate for our locations through referral management systems
* Working with hospital discharge planners and families to arrange admission
* Ensuring all required documents are received prior to admission
* Verifying insurance benefits
* Onsiting patients at the hospital
* Attend hospital meetings
* Fill in for Central Intake PRN

What We're Looking For (Your Superpowers!):
* A current, unencumbered clinical license in Michigan. OR, if you're a non-clinician, you bring five or more years of experience rocking it as a hospital liaison or in a central intake role.
* Three-plus years of referral management experience? That's a huge plus!
* A solid understanding of what makes a great admission for skilled nursing and long-term care.
* Off-the-charts organizational skills and an eye for the tiniest details.
* Outstanding phone-based communication skills that are both brilliant and empathetic, you're a master conversationalist!
* The ability to work independently, prioritize like a pro, and handle a high volume of admissions with grace under pressure.
* Tech-savvy with electronic health records (EHR) and admissions software.
* Flexibility to work rotating weekends and holidays.
