LMSW Case Manager

Kaiser Permanente is seeking an LMSW Case Manager to join our Community & Social Services team in Healthcare & Medical Services. In this role, you’ll provide psychosocial assessments, develop person-centered care plans, and coordinate medical, social, and community resources for diverse patient populations. You will collaborate closely with physicians, nurses, and behavioral health providers to support safe discharges, reduce readmissions, and enhance quality of life. The ideal candidate brings strong clinical judgment, excellent documentation skills, cultural humility, and a passion for advocacy in fast‑paced, interdisciplinary settings.
