The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and, through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
We are seeking a compassionate and experienced Charge Registered Nurse (RN) to support the daily coordination and delivery of clinical care services for our participants. This role provides clinical leadership, oversees day-to-day nursing operations, and collaborates with interdisciplinary teams to ensure high-quality, participant-centered care.
Duties and Responsibilities
* Coordinate and oversee daily clinical care activities and workflows.
* Provide leadership and support to the clinical care team.
* Monitor and manage participants' acute and chronic health conditions in collaboration with Primary Care Providers.
* Administer medications, treatments, immunizations, and wound care as prescribed.
* Conduct nursing assessments and contribute to care planning.
* Assist with staff onboarding, training, and development.
* Maintain clinic inventory, equipment, and medication management processes.
* Support quality, compliance, and audit readiness initiatives.
* Participate in after-hours on-call and holiday rotation as needed.
Qualifications
* Current Registered Nurse (RN) license and graduate of an accredited nursing program.
* Two years of Charge Nurse or leadership experience preferred.
* At least one year of experience working with frail elderly, long-term care, or similar populations.
* Current CPR certification (AHA) or ability to obtain.
* Strong clinical judgment, communication, organizational, and leadership skills.
* Commitment to compassionate, participant-focused care.
