Registered Nurse – Behavioral Health

Full-Time | Day Shift | Worcester/Leicester Area | $5K Sign On Bonus!

What You’ll Do

As a Behavioral Health RN, you will:

  • Assess patients’ physical, behavioral, psychosocial, and environmental needs.

  • Develop, implement, and update individualized plans of care.

  • Manage an assigned caseload and coordinate services with physicians, caregivers, facilities, and the interdisciplinary care team.

  • Complete initial evaluations, ongoing assessments, and follow-up visits based on each patient’s needs.

  • Monitor changes in condition and communicate concerns promptly to the appropriate care team members.

  • Educate patients, families, and caregivers about behavioral health needs, medications, symptom management, and the plan of care.

  • Provide clinical guidance and supervision to LPNs and home health aides as appropriate.

  • Maintain accurate, timely clinical documentation.

  • Identify and report safety concerns or other issues that may affect patient care.

What You’ll Need

  • Current, unrestricted Massachusetts RN license

  • Associate degree in Nursing required; bachelor’s degree preferred

  • At least one year of nursing experience in a clinical care setting

  • Behavioral health nursing experience preferred

  • Home health or hospice experience preferred

  • Homecare HomeBase experience preferred

  • Ability and willingness to travel throughout the Worcester/Leicester service area

  • Dependable vehicle, valid driver’s license, and current auto insurance

  • Ability to lift, push, pull, support, or transfer up to 50 pounds
