Home Health Physical Therapist Assistant (PTA)

EHS Home Health Care Services, Inc., part of Advocate Health, is seeking a Physical Therapy Assistant to support patients across the South and Southwest suburbs. Under the direction of a licensed physical therapist, you will provide in-home rehabilitation that helps patients recover from illness or surgery and manage chronic conditions safely at home. Responsibilities include assisting with prescribed exercises and mobility activities, monitoring patient response and safety, educating patients and caregivers, documenting care, and coordinating with an interdisciplinary team.
